What services are included in your post-hospital discharge support?

We provide hands-on, home-based services after discharge, including medication reminders and dose checks, assistance with activities of daily living (bathing, dressing, meals), wound and post-op care (dressing changes, incision checks), safe transfer and mobility support, fall-prevention measures, accompaniment or transport to follow-up appointments, telehealth assistance, and visit documentation / clinical reporting.

How do you coordinate with the hospital or discharge planner?

After we receive the discharge instructions, our Area Manager reviews the clinical orders, performs an in-home assessment, and matches a caregiver who fits the clinical and personal needs. We keep the hospital, discharge planner and family informed with timely reports and escalate any concerns immediately so clinical teams have the visibility they need.

How quickly can care begin after discharge?

We prioritize rapid starts for discharge cases. When possible we arrange caregiver coverage within 24–48 hours; for urgent needs we will expedite and work directly with the hospital care team to arrange immediate support. Exact timing depends on caregiver availability and the complexity of clinical instructions.

Will insurance or Medicare cover these services?

Coverage varies by payer and service type. Some clinical nursing or therapy visits may be billable under insurance or Medicare rules; many personal care, companion hours and transportation are typically private-pay. We assist families with benefit verification and can help explore payer options for a specific patient.
